Completing the forms

The Notified Absence form and the Authorised Absence Request form are completed electronically.

The Authorised Absence Request form allows supporting evidence (for absences longer than 5 consecutive working days or for evidenced requests for retrospective absence) to be uploaded to the form. You cannot save a partly completed form, so make sure you have your evidence ready before starting to complete the form.

1. Fill in the online form, ensuring all compulsory information is completed

2. Upload supporting evidence if required. You will need to have electronic copies of the evidence available

3. You will receive an e-mail confirmation and copy of your form

Please note: As part of this process Student Administration may need to contact you to require further information related to your absence – this will be assessed on a case-by-case basis.

 

Authorised Absence Request Form and Supporting Evidence

The Authorised Absence request form is an online form which should be completed for absences of more than 5 consecutive working days or where you have not been able to notify us of an absence of less than five working days by the end of the day it occurred.

The form must be submitted to Student Administration either before your absence or within five working days of the end of the period of absence. Failure to do so may result in the absence not being approved and counting as a missed point of contact.

If your absence is for longer than five working days, you need to submit supporting evidence with your form. You can check the type of evidence required in the table below. Please note, Student Administration reserve the right to request to see the original evidence in order to authorise an absence.

For Absence of 5 working days or less, no evidence is required. Students should submit the Notified Absence Form if possible in advance of the absence. Where it is not possible to notify us of your absence in advance, you should do this on the day the absence occurred. If you do not do this by the end of the day it occurred,you must sumbit an Authorised Absence form. You cannot submit a request for retrospective notification of absence. However, Student Administration may require students to produce evidence in instances of repeated Notified Absences.

Bereavement

Copy of death certificate, confirmation of funeral or burial arrangements or a note from family member to verify situation. 

Documented personal problems

Illness of dependent or family member

Medical Certification and statement explaining illness and why personal attention is necessary
Bereavement

Death certificate, confirmation of funeral arrangements or a note from family member to verify  situation. 

Severe transport problem

A copy of online or newspaper reports on the problem 

Court attendance

Official correspondence from the Court confirming attendance requirement

Victim of crime

Statement of events, police report and crime reference number

Professional Development

Confirmation of invitation to interview, taster day etc.

Involvement in a significant/prestigious event

Letter of invitation from the relevant organising body confirming the requirement to be available on specific dates, confirmation of travel and or event booking.

Sport commitment at national/county level

Official correspondence from the relevant sporting body confirming the requirement to be available on specific dates, confirmation of travel and or event booking.
